For individuals currently hospitalized, please contact our Admissions/Social Services Department as soon as possible. We will follow up with the hospital case manager for assessment and discharge planning.
For individuals living at home, in assisted living, an independent living facility, or another long term care facility, Hooverwood's Admissions/Social Services staff will be happy to discuss the admission process with you.

Social Services
Our team of social workers welcomes new residents and families, offers supportive services, coordinates Care Plan meetings and in-house transfers, and assists with discharge planning. They are available for other non-medical needs as they arise. A social worker is assigned to each of Hooverwood's five units. To facilitate good communication between families and staff, each social worker has 24-hour confidential voice mail and may also be reached via e-mail by clicking on the link for your family member's unit below.
